Perinatal Mood Disorders: Beyond Postpartum Depression

When people think of maternal mental health, postpartum depression usually comes to mind first. But the spectrum of perinatal mood and anxiety disorders (PMADs) is much broader—and many mothers suffer without realizing they have a treatable condition.

Types of Perinatal Mood Disorders

PMADs can occur during pregnancy or anytime in the first year after birth. They include prenatal depression, which affects up to 15-20% of pregnant women but often goes undiagnosed because symptoms are attributed to normal pregnancy discomforts. Postpartum anxiety affects about 10% of new mothers and involves persistent, racing worry that something terrible will happen. Postpartum OCD causes intrusive, unwanted thoughts about harm coming to the baby, which can be deeply distressing. Postpartum PTSD can follow a traumatic birth experience, affecting up to 9% of mothers. Postpartum psychosis is rare but serious, requiring immediate medical attention.

Risk Factors to Be Aware Of

While PMADs can affect anyone, risk factors include a personal or family history of depression or anxiety, lack of social support, relationship difficulties, financial stress, pregnancy complications, a history of infertility or pregnancy loss, and being a first-time mother. Awareness of these risk factors empowers you to seek support proactively rather than reactively.

Treatment Is Available and Effective

The most important thing to know is that PMADs are treatable. With the right combination of therapy, support, and sometimes medication, the vast majority of mothers recover fully. Cognitive Behavioral Therapy and interpersonal therapy are both highly effective. Support groups can also be invaluable.
